Transitions offers a specialized Parent/Guardian involvement support and training program when requested by the parent or guardian for additional training and support. This service can be provided clinic based or home based. Our outreach team works collaboratively to ensure that treatment goals align with family values and priorities. - Intensity and Duration: Our program is offered 3-6 hours per day Monday-Friday in our clinic.

Training and collaboration empowers families to support learning and manage behavior outside of therapy sessions.

The EIBI Process: From Assessment to Implementation
- Initial Assessment: The process begins with a comprehensive evaluation of the child’s strengths, needs, and current levels of functioning, often through standardized tools, observation, and caregiver interviews that occur with our Licensed Social Work Team.
- Goal Setting: Specific, measurable, and meaningful goals are established with our Licensed Social work and Behavioral Health Team. These goals may target communication, social skills, play, self-help, or reduction of challenging behaviors.
- Implementation: quality mental health specialists as well as your child’s therapy team work collaboratively to implement the goals that have been established by the parent and Transition’s treatment team.
- Ongoing Monitoring: Regular review meetings ensure that progress is tracked and adjustments are made as needed.
